St. Mary’s University students with approved accommodations at the start of each new term, fill out the Accommodation Registration Form that is emailed to you to activate supports for that term’s classes. Use the form to:
- Indicate which courses you want accommodations for
- Confirm which accommodations you plan to use
- Confirm which courses you do not plan to use accommodations for
- Please submit the form for each course you are registered for
- Unless indicated otherwise when completing the form, we will assume you want accommodations in each course and communicate that to each of your professors
Booking Test Accommodations
Once you’re registered, you can book your test online through the Accommodated Testing Request Form.
You must:
- Book at least seven (7) business days in advance of your test
- Submit a separate form for each test you need to write
- Tests must be written at the same time as the class unless alternate arrangements have been approved in advance.
- When booking, please include:
- Course name and instructor
- Date and time your class is writing
- Accommodations you plan to use (e.g., extra time, computer)
- Speech to Text Dictation Accommodation (STT): If you need Speech to Text for your test and have that as an accommodation, you must book your test seven (7) days in advance to arrange space accommodations
- Memory Aid: If you plan to use a memory aid, please connect with Jenna about the memory aid procedure at jenna.walker@stmu.ca. A Memory Aid must be submitted three (3) days before your test to be reviewed and approved by your instructor
You must write your test or quiz on the same day as the rest of your class. If you need to take your test or quiz on an alternative day due to a testing conflict or a disability-related reason, you must obtain permission by emailing your instructor with the email access@stmu.ca in CC.
Writing your Accommodated Test
Here’s what you need to know when showing up for your accommodated test:
- Entry: Please enter through the testing door (C114) located next to the counselling waiting room.
- Devices and Bags: You’ll sign in at the desk the testing center assistant. Be sure to turn off your phone and any electronic devices and place your bag. You can keep them in our secure cubby area by the front door.
- What to Bring: Bring your student ID and the materials your professor has indicated (e.g., pens, pencils). Your instructor will let us know if there are specific items you can or cannot use.
- Timing: We understand you’ll be arriving just before the start time. We’ll make sure you’re set up as quickly as possible so you can maximize your exam time.

What Is Schedule 4?
Schedule 4 is the form that confirms a permanent disability, so you can access:
- Canada Student Grant for Students with Disabilities – up to $2,800/loan year
- Canada Student Grant for Services & Equipment – up to $20,000/loan year
- Alberta Grant for Students with Disabilities – up to $3,000/loan year
• Receipts for services or equipment must be uploaded within 30 days of your study period ending.
1. Confirm Eligibility
You qualify if you:
- Are a Canadian citizen or permanent resident living in Alberta
- Are enrolled in a loan-eligible program full-time (or ≥ 40% course load with disability)
- Have documentation of a permanent disability
- Demonstrate financial need (even $1 of assessed federal need)
2. Book Your Schedule 4 Meeting
Use our online scheduler to set up a meeting with Accessibility Services. Bring:
- Your disability proof (assessment or verification form)
- Your Alberta Student Aid ID
- A copy of your loan/grant application (optional)
3. Complete & Submit Schedule 4
In your meeting, we’ll:
- Fill out the Schedule 4 form together
- List requested supports (tutoring, tech, software, equipment)
- Submit the form to Alberta Student Aid
You’ll receive a Student Award Letter detailing approved items and amounts.
4. Submit Your Receipts
- Upload receipts as soon as you purchase
- Must be in by 30 days after your study period ends
- Keep copies for your records

- You need a new Schedule 4 each loan period (Fall, Winter, Spring)
- Plan ahead: submit equipment or software requests at least 90 days before term end
